BICEPS TEAR

Tear versus Rupture

First, a patient with a small partial biceps muscle body tear, on an anti-platelet.
Note that despite the swelling and ecchymosis the bicep muscle bulk is in its natural position:

His US showed intact biceps muscle, and subsequent vascular studies to rule out Paget- Schroeder were normal.

A full biceps long tendon rupture, however will reveal a Popeye Sign on the effected side:

You can see how the muscle bulk has retracted distally from the origin site, giving a large trough appearance between the deltoid and biceps muscle bulk.
